All registrations must have a completed Cedar Canyon Camp Liability Release form submitted for EVERY adult and child that is being registered. Use the button below to access the .pdf form. There are two options for completing and submitting:

1. ELECTRONICALLY: Download and open with Adobe Acrobat Reader. Complete the form on your computer, type your full name in the signature field, and click the submit button to send the form via email attachment – it will go directly to the camp.
2. PRINT AND MAIL: Print, sign, and return via postal mail to Angela Hovland, 10777 Pass Creek Rd., Edgemont SD 57735.

2. Enter your information into the dynamic forms and click Submit Registration button.
3. When successfully submitted, you will receive a confirmation message on the registration page, and you will also receive an email* confirmation of your registration
   (Please check spam folder and any spam filters if you do not receive the confirmation email within a few minutes of successful submission.)
4. Complete a  Liability and User Agreement for each person attending.
5. Send a check to Susan Paulton, 10666 Pass Creek Road, Edgemont, SD 57735. (Make check payable to Angela Hovland w/ Black Hills Mini Shindig in the memo.)

Remember EVERYONE attending the Black Hills Mini Shindig MUST fill out a Liability and User Agreement.

   (By not clicking the Submit Registration button, NO information will be transmitted through the website – it will only remain local on your computer.)
3. Use your browser Print function (or press Ctrl-P) to print the completed form.
4. Complete a  Liability and User Agreement for each person attending.
5. Send the printed registration form, the printed Liability and User Agreement forms, and a check to Susan Paulton, 10666 Pass Creek Road, Edgemont, SD 57735. (Make check payable to  Angela Hovland w/ Black Hills Mini Shindig  in the memo.)
6. You can also scan the completed forms and email them to   vernhovland@aol.com.
